If you want to be part of a dedicated group of guys making an immediate, direct and positive impact on the lives of our neighbours and want 100% of your donations to go directly to a local charity, join us.

By completing the registration form below, I understand that I am making a commitment to attend up to a maximum of four meetings a year and donate $100 on each occasion to the chosen charity. 

​

All proceeds will be given directly to local charities serving the communities of Southern Georgian Bay. I agree to fulfill my donation commitment even if I did not vote for the charity selected by majority vote or attend the meeting at which the voting took place.

REGISTRATION & COMMITMENT FORM

COMMITMENT

With my signature below, I am agreeing that the information I provide below is accurate and true. I am pledging to participate in 100 Men Who Care – Southern Georgian Bay, and I am making a  personal commitment to contribute to a maximum of $400 each calendar year ($100 per meeting to a maximum of four meetings) to local nonprofit organizations serving the Southern Georgian Bay region. I agree to donate each meeting to the nonprofit organization selected by the group’s majority vote. If I am unable to attend a meeting, I  will still provide my donation to the selected charity. I also acknowledge that photographs and videos taken at events and meetings may include my image and may be used in promotional materials for 100 Men Who Care Southern Georgian Bay.

I understand my personal contact information is strictly confidential, and I understand it will not be shared or distributed to an outside third party without my expressed consent.

If 100 Men Who Care Southern Georgian Bay chooses to publish a Membership Directory, I agree that my contact information be included in that directory.

OFFLINE REGISTRATION

Completed Commitment Forms may be scanned and sent via e-mail to 100mensgb@gmail.com or forms may be completed and turned in at a meeting. Should you wish to discontinue membership at any time  after your four-time commitment, please send an e-mail to 100mensgb@gmail.com indicating your  withdrawal.
